Immunization

All students applying to LUA must have a valid Kentucky immunization certificate which must be presented by the first day of classes.

A second Measles, Mumps and Rubella (MMR) vaccine is required upon entry into school. Hepatitis B vaccine is required for children entering school who were born after October 1, 1992 or later.

Children younger than 5 years-of-age need a HIB vaccine.

All students entering the sixth grade are required to have a second Measles, Mumps, Rubella (MMR) vaccination prior to the beginning of school.

As of March, 2003, a tetanus-diphtheria booster is now required for those between age 11 and 12 if it has been at least five (5) years since their last booster. This is in addition to the second MMR, hepatitis B series, and physical examination requirement. Sixth grade students not meeting these requirements will not be allowed to begin school.
